HUBUNGAN KADAR GLYCOSYLATED HEMOGLOBIN (HBA1C) DENGAN KEJADIAN AMPUTASI EKSTREMITAS PADA PASIEN ULKUS KAKI DIABETIKUM DI RSUD ABDOEL WAHAB SJAHRANIE SAMARINDA

Abstract

Diabetic Foot Ulcers (UKD) are ulcers on the feet caused by neuropathy. The prevalence of UKD sufferers in Indonesia is around 15% with an amputation rate of 30% and a mortality rate of 32%. UKD is known as a chronic wound with a long healing time. HbA1c levels can be used as a status parameter for DM diagnosis because there is a strong relationship between HbA1c levels and blood glucose during the lifetime of red blood cells. The majority of diabetics have a higher percentage of HbA1c levels than non-diabetic sufferers. HbA1c levels are one way to evaluate blood sugar control. This study aims to determine the relationship between HbA1c levels and the incidence of amputations in UKD patients at Abdoel Wahab Sjahranie Hospital, Samarinda. This research design is analytical observational with a case control approach. The research data came from medical records of UKD patients at Abdoel Wahab Sjahranie Hospital Samarinda in 2019–2023 using a purposive sampling method. There were 66 patients who met the inclusion and exclusion criteria. Univariate and bivariate analysis used the Chi-Square test. The results of the study showed that the Diabetic Foot Ulcer (UKD) patients in this study were mostly in the 41-60 age group with the case group (66.7%) and the control group (60.7%), the largest gender being men with case group (48.5%) and control group (63.6%), Hba1c levels were controlled (≤7%) in 9 patients (27.3%) in the case group and 20 patients (60.6) in the control group. Meanwhile, UKD patients who had uncontrolled Hba1c levels (>7%) were 24 patients (72.7%) in the case group and 13 patients (39.4%) in the control group. Patients with uncontrolled HbA1c levels are 4.103 times more at risk of experiencing limb amputation compared to patients with controlled HbA1c levels. The results of statistical tests regarding the relationship between Hba1c levels and limb amputation showed p = 0.006; OR = 4.103. It can be concluded that Hba1c levels have a significant relationship with the incidence of limb amputation.
